In recognition of National Case Management Week, we celebrate the dedicated professionals who guide our patients through every step of their care journey.

This year’s theme, “The Heart of Collaborative Care,” perfectly describes our incredible Case Management teams at SIH Memorial Hospital of Carbondale, SIH St. Joseph Memorial Hospital, SIH Herrin Hospital and SIH Harrisburg Medical Center.

Our case managers are the heart of the care journey, connecting patients, families and healthcare teams to ensure every individual receives the support and resources they need for the best possible outcomes.

Our team has been busy this week! They proudly joined local celebrations across the region!

SIH St. Joseph Memorial Hospital thrived at the Murphysboro Apple Festival, bringing home a few awards:

1st Place & Most Beautiful Float - Grand Parade
1st Place - Business Category, Children's Pet & Hobby Parade
2nd Place - Hospital Lobby Window Display

Meanwhile, team members from SIH Harrisburg Medical Center, SIH Primary Care Harrisburg and Eldorado came together to march in the Harrisburg High School Hometown Heroes Homecoming Parade.

And the festivities keep going - look for our SIH Herrin Hospital crew in the Herrin High School Homecoming Parade next weekend!

This incredible group was recently recognized with a “Star Celebration” for their outstanding commitment to sepsis care. Their consistent excellence with the sepsis bundle core measure is saving lives and setting the standard for quality care.

To mark the achievement, the team reviewed sepsis data, received handy resource tools for their badges, and celebrated their patient and team wins with star-themed treats.
