Caldwell Home Services would like to extend a special, thank you to all of our clients who without them we would be nothing. It is an honor and a privilege to be able to serve the people in Caldwell County and to help them on a daily basis. Jesus has called us to be servants and I have to say a special thank you to our employees as well because it takes a special kind of person to truly love and show compassion to complete strangers. In home care has given us the opportunity to be able to go out into the community and spread the love of Christ.

Today we fed over 120 people in our community. Being the hands and feet of Jesus is our mission.

We are so happy to finally announce that Caldwell Home Services has contracted with VAYA Health and can now provide the Medicaid 1915(i) Services which Includes:

*(CLS) Community Living and Supports
*Respite
*Community Transition
*Individual Placement and Support – Supported Employment
*Individual and Transitional Support

These services are available for people with mental health conditions, substance use disorders, intellectual or developmental disabilities, or traumatic brain injuries.

Conduct Intake Assesments
Create client plan of care and task sheets
Supervise caregivers
Complete orientation for new hires
Conduct annual skills check with caregiver
Coordinate with client case managers
