A frequently asked question we get is, how should I prepare for my appointment?
When coming to your appointment please bring your I.D., insurance cards, and an imaging disc if necessary*.
If you are a new patient we ask that you arrive 10-15 minutes early to fill out paperwork.
If you are an established patient and your insurance has changed since your last visit, please call us before your next appointment to let us know as we may need to get an authorization before we can see you again!

*We will let you know if you need a disc for us to view your images, if you have any, when making your appointment.
