Thinking about a multifocal lens for cataract surgery?

These lenses can help you see at multiple distances—but they also come with trade-offs like halos, glare, and slightly reduced contrast.

In this quick video, Dr. Plummer explains who tends to love them… and who may be better off with a different option.

Night and day improvement! This patient has horrible dry eye from chemotherapy side effects. We placed amniotic membranes over both eyes to heal the surface. The first two photos show severe surface damage, which causes pain and blurry vision. The second two photos show that damage virtually disappear. The patient said vision and comfort were both much better. This technology really works!

It’s always special when patients bring us gifts, but I always get a little misty-eyed when they say “I can make things like this because you gave me my vision back”. We are so lucky to have such talented and generous patients. Being able to give people clear vision through cataract surgery is such a joy!

Eye Caramba! Most people don’t think about the eyes being affected by the shingles virus. In this case, you can see viral lesions highlighted in green by fluorescein dye. This lesion was nearly invisible without this dye. Most patients improve with antiviral treatment, but this shows why it’s important to get your shingles vaccine if your primary care doctor recommends it.

Technology is awesome. We take measurements of the eye in the office, and the computer matches those measurements to the surgical microscope overlay in realtime, allowing me to precisely align the lens implant to best correct astigmatism. For someone who has worn glasses their whole life, this is a game changer! Imagine waking up and being able to see everything without having to put on glasses. Precision technology makes it happen.

Light from the microscope is reflecting back through the iris and shining through some patchy areas. This condition is called pigment dispersion syndrome and happens when the iris chafes on other ocular structures, releasing pigment into the eye and leaving behind thin clear patches. This sometimes leads to glaucoma, though thankfully not in this case.

This patient was referred to me by their primary care doctor for an epiretinal membrane, which is scar tissue covering the retina. Their office has a camera that takes a picture like this one, and that picture is either reviewed by a remote telehealth service or by AI to screen for things like diabetic eye disease. The patient was understandably scared by this news, but the yellowish sheen that you can see at the center of the photo is not an ERM, it’s a normal finding in young healthy patients. It just goes to show that screening tools can be useful, but nothing beats an honest in-person exam by an ophthalmologist.

20/20 vision on the first day! This patient had a macular hole in the right eye (notice in the second photo how the left side images look thinner and more ragged). Normally, this condition would have excluded the patient from having premium lens implant technology. Fortunately, Dr. Jonna at did an amazing job repairing the defect and achieved a nearly perfect anatomical outcome. This is why it’s so important to customize a treatment plan for every patient. While it’s true that many retina conditions make it difficult to achieve the best outcomes with premium lens implants, I’ve had many cases like this where the vision was even better than we hoped for. The key thing is taking the time to get to know each patient and their goals, and to make sure they understand the limitations and have reasonable expectations. Technology keeps getting better and better, allowing us to offer glasses free vision to more of our patients.
