Coal Creek Post Acute & Assisted Living is among the elite 19% of SNFs that earned a “High Performing” rating, the highest possible achievement. You can see the facility’s ratings, along with related data and descriptions, on its U.S. News Profile page.

A High-Performing rating means that a home is eligible to promote its award by displaying a U.S. News-trademarked Best Nursing Homes award “badge” (logo) on websites, social media, advertisements, signage, recruiting materials, video, mailings, plaques, and other promotional media. Among them, our dietary team stands out as one of the most hardworking and committed groups. They are often the first to arrive and the last to leave, ensuring our residents receive nutritious, well-prepared meals every day. Meet Chante! She is a talented therapist, valued coworker, devoted mom, dog mom, and skilled magnet maker.

My name is Chante I’m 36 years old. I’ve been a COTA (certified occupational therapy assistant) at coal creek for 8 years. I have a background as a CNA. I chose the therapy path because I enjoy playing a role in helping patients regain their strength and ability to go home and successfully return to their everyday life enjoying time with their family and friends.
