Graduate of an:
Accredited College/School of Nursing, Graduate School of Social Work.
Possession of a Bachelor's degree in Nursing, Behavioral Health or appropriate health related filed required, Masters preferred.
State License:
Current state licensures as RN, LCSW, and LPC preferred.
Previous program management experience or acute care hospital experience.
CPR Certification
First Aid Certification
Microsoft knowledge/Computer proficiency

Job description

Provide advanced nursing care and treatment to patients. Perform physical examinations, order diagnostic tests, develop treatment plans and prescribe drugs or other therapies.

Tasks:

· Diagnose or treat acute health care problems such as illnesses, infections, and injuries.

· Analyze and interpret patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.

· Order, perform, or interpret the results of diagnostic tests such as complete blood counts (CBCs), electrocardiograms (EKGs), and radiographs (x-rays).

· Counsel patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over-the-counter (OTC) medications, and herbal remedies.

· Educate patients about self-management of acute or chronic illnesses, tailoring instructions to patients' individual circumstances.

· Diagnose or treat complex, unstable, comorbid, episodic, or emergency conditions in collaboration with other health care providers as necessary.

Provide high quality patient care in the areas of treatment planning, assessments, individual/group/family treatment, discharge planning, counseling, and providing education to psychiatric patients, families, and significant support systems in a timely and cost-effective manner. This position will be responsible for planning, organizing, developing, and directing the overall operation of our hospital’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Executive Director, to assure that the medically related emotional and social needs of the patients are met/maintained on an individual basis. Additionally, you will be expected to function as a member of the multi-disciplinary team and participate in the unit community relations program.

Must be at least 21 years of age.
Requires a Master of Social Work degree
May require an active, unencumbered licensure as an LMSW or LCSW if required by the state
Must be CPR/BLS certified or able to obtain certification prior to working
Ability to maintain cooperative working relationship with patients, patients’ families, and medical staff, as well as facility personnel. Possess leadership qualities.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Complete the initial nursing assessment within 8 hours of admission. Complete an assessment for withdrawal symptoms upon admission within a reasonable amount of time and on an ongoing basis. Reassess the patients on an ongoing basis, based on patient needs, interventions, and expected outcomes and is documented.

Participate in and receive the nursing report upon report for duty. Report any abnormal changes in patient health promptly. A clear, concise, and accurate report based on the nursing process is reflected in the patient record and change of shift report. Make daily rounds of the unit to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Report any findings to the Nursing Manager. Notifies attending physician and family when the patient becomes ill or is injured.

Participates in the multi-disciplinary care planning and implementation to meet the physical, psychological, psychosocial, and spiritual needs of the patient and their family. Initiate the development of the interdisciplinary plan of care and manages nursing actions/interventions to address the patients ongoing and long-term needs based on the nursing assessment, the DSM diagnoses, strength and weaknesses, estimated length of stay, discharge plan, problems to be addressed, and measurable goals with a target date and interventions.

Develop work assignments, and/or assist in completing and performing tasks. Assist in developing methods for coordinating nursing services with other patient services to ensure adherence to prescribed plan of care. Provide nursing care based on the standards of care, assist the patient to achieve maximum psychological and health capabilities and promote achievement of expected outcomes at discharge.

Evaluate patient’s actual and/or potential response to the disease process, interventions and the outcomes of nursing care and multi-disciplinary treatment of care services. This includes diagnostic and problem solving, demonstrating critical thinking skills, ability to prioritize and evaluate patients and their response to treatments, and interventions, and preparing nursing notes accurately documenting patient responses/outcomes to the plan of care, interventions, and expected outcomes at discharge.

Utilize performance improvement processes to address issues that arise related to patient care, appropriateness of care, and use of technology and efficiencies. This includes identifying and resolving patient care, unit and/or organizational issues in a timely manner and documenting appropriately.

Establish and maintain positive interpersonal relationships and promotes effective teamwork with patients, families, customers and the community, including demonstrating a professional, caring, and service-oriented attitude in all interactions, willingly collaborating and assisting all members of the health care team, and actively participating in relevant meetings, rounds, and/or teams as assigned.

Provide patient and family education based on readiness and the ability to learn, priorities of the patient’s multi-disciplinary needs, utilization of appropriate resources and expected outcomes at discharge. Maintain privacy, security, and confidentiality of patient, staff and organizational data and/or information. Use leadership skills to serve as a role model and enhance the delivery of care and services and the overall operations of the work environment.

New research led by University of Pittsburgh psychologists pools data from dozens of studies to answer these questions, showing that older adults may be able to prevent declines in a certain kind of memory by sticking to regular exercise.

Regular exercise may help reduce declines in episodic memory for older adults.

Provides assessment and counseling to psychiatric patients, their families, and significant support systems

Assesses and provides discharge planning services to psychiatric patients within assigned unit.

Provides clinical supervision to other Social Workers and facilitates treatment team.

Conducts initial psychosocial assessment interview with patient within 72 hours of admission.

Documents barriers to completion of assessment if applicable and places completed assessment within the medical record.

Completes psychosocial evaluations on assigned patients in a timely fashion. Displays good interviewing, diagnostic and report writing skills

Conducts both individual and group therapies for patient education related to but not limited to: disease process, emotional management, stress management, coping skills, decision making and life skill development.
Identifies and develops a plan of care related to patient’s emotional needs, community support systems and on-going clinical concerns.

Must be at least 21 years of age.

Requires an active LCSW or LMSW.
