Take home point of a complicated topic. Frailty (fatigue, multiple medical diagnoses, weakness, multiple meds, falls) + memory changes = ExPoNeNtIaL effect!! Leading to more rapid decline and increase in needs

Frailty + dementia = a powerful combination that affects so much more than memory.

A systematic review showed that when someone living with dementia is also frail, they’re at higher risk for hospitalizations, faster decline, and losing independence.

This is why I don’t just evaluate cognition… I look at strength, mobility, safety, nutrition, daily function, and caregiver support. Treating memory alone isn’t enough.
Proactive, in-home, whole-person care can prevent crises and change the path forward.

✨ When we identify frailty early, we can support families and slow the decline.

It’s not just about memory. It’s about keeping your loved one strong, safe, and supported.

Sometimes it’s not a diagnosis—it’s the subtle changes in an aging loved one.

People living with dementia often experience disruptions in their sleep patterns. Also common is sundowning, which is a pattern of increased confusion and behavioral changes in the late afternoon and evening. I am dumbfounded, aghast, gobsmacked and in complete disbelief by this list. As a physician, in residency and through 20+ years of practice, I would not be who I am without the mentoring and direction of the nurses I am privileged and honored to have worked with in my training and career. Not to mention the therapists who are a sounding board and the educators who got me to this degree. Social workers?? Where would ANY patient or health system be without your tireless efforts to improve every aspect of our community. Truly unconscionable. You all have my utmost support. We need one another.
