🌟 Hospice Case Manager Jill P., RN 🌟

🩺 42 years in nursing | 4 years in hospice care

Hospice nursing is more than a profession – it’s a calling. For Jill, that calling came from a deeply personal experience.

“Five years ago, I had a friend who passed away from cancer. I was exposed to hospice care at that time and watched my friend suffer emotionally and physically during her last year of life. I felt a calling to assist patients and their caregivers with pain management and other end-of-life symptoms. It is my mission that end-of-life should not be awful, but hopefully more peaceful and calmer.”

Jill brings over four decades of nursing experience to her role, but it’s her empathy and dedication to easing suffering that truly define her work. 💜

🦃Thanksgiving is almost here!

This year, one of our newest Home Health Nurses, Lina M., RN, is celebrating Thanksgiving for the first time in over 15 years. Prior to joining our Saint Joseph VNA Home Care team in Indiana, Lina spent 13 years in the Amazon in Brazil with a nonprofit organization providing medical and dental care to remote communities.

We’re thankful for Lina – and for all our amazing colleagues who make Trinity Health At Home a family. 💟

👟Our Saint Francis at Home team in Delaware laced up their sneakers and joined the Walk to End Alzheimer’s – a cause near and dear to our hearts at Trinity Health At Home.

We’re so grateful for colleagues who go above and beyond, not only caring for patients and families every day but also stepping out as advocates for hope, research and a future without Alzheimer’s.

🥫🍞 Filling Shelves, Warming Hearts! ❤️🤝

Colleagues from Mercy Home Health and St. Mary Home Care and Hospice in Pennsylvania teamed up for an annual food drive supporting Mercy Neighborhood Ministries.

Together, they collected cereal boxes, canned goods and other nonperishables to help stock the Mercy Meals program – providing essential food to neighbors facing food insecurity.

Huge thanks to every colleague who contributed! Your kindness and teamwork make a real difference in the communities we serve. 👏
