📝 Job Overview:

We are seeking licensed Physical, Occupational, and Speech Therapists to join our per diem homecare rehab team serving Mercer County, NJ. This flexible, field-based position allows you to provide individualized care to patients in the comfort of their homes—on your schedule.

👩‍⚕️ Responsibilities:
• Conduct comprehensive in-home assessments
• Develop and implement personalized treatment plans
• Provide skilled therapy services according to patient goals and care plans
• Educate patients and caregivers on safe practices and home exercises
• Maintain accurate and timely documentation (EMR system)
• Coordinate with nurses, physicians, and other therapists to ensure continuity of care

✅ Requirements:
• Active NJ license in PT, OT, or ST
• Minimum 1 year of experience (Home health experience a plus)
• Reliable transportation and valid driver’s license
• Strong clinical, organizational, and communication skills
• EMR documentation experience preferred (training available)
