Job Title: Designated Coordinator and PCA Supervision

Salary: $70,000 - $90,000 per year, depending on experience

Schedule: Monday - Friday, 9am - 5pm

Job Summary:

We are seeking a highly skilled and experienced Designated Coordinator and PCA Supervisor to join our team. The successful candidate will be responsible for leading our 245D program, training staff, and supervising Personal Care Assistant (PCA) services. This is a full-time position that requires travel for home visits.

Responsibilities:

- Create and implement (CSSP-A) for 245D programs
- Train staff on program requirements and ensure compliance
- Supervise PCA services, including conducting home visits to monitor quality of care
- Develop and maintain accurate records and reports
- Ensure program goals and objectives are met

Requirements:

- 2+ years of experience as a 245D Designated Coordinate or supervision
- Strong leadership and communication skills
- Ability to travel for home visits (valid driver's license and reliable transportation required)
- High school diploma or equivalent required; degree in healthcare or related field preferred
