Job Description: The position is for providing case management services to high-risk, high-utilizing members with acute and/or chronic psychiatric conditions. The job involves developing treatment and care plans for this specific population across the continuum of care. The role requires helping transfer patients to and from The County. Although the job has been designated as "inpatient case manager," it's essentially for our Medi-Cal patients. On September 25, 2022, Governor Newsom signed AB 1759. This bill will require applicants for licensure and licensees to take a one-time course on telehealth.
