Due to hazardous winter storm conditions, all WCHC locations will be CLOSED on Monday, February 23. We will resume our normal working hours on Tuesday, February 24.

Along with proper diet, medication, and exercise, regularly monitoring your blood pressure and glucose levels is essential for managing chronic hypertension and diabetes.

Our Remote Patient Monitoring Service helps patients who could benefit from ongoing monitoring achieve their health goals and stay connected to their care team. Program participants are given a monitoring device that they can use at home. This home device directly connects to their health file on our systems so that our medical providers can have a continuous record of readings and attend to patients’ needs in real-time.
