We guides families in real time, as their needs evolve, which involves assessment, specialized recommendations for care based on the needs of each individual and ongoing monitoring and communication. Assessment: For each new client, Care NOLA conducts a comprehensive assessment of needs, identifying immediate concerns. Focusing on client resources, urgency and overall goals, solution-based strategies are developed.

​Care Plan: Care NOLA provides regular client visits to address agreed upon care management activities. Implementation: Our belief is that consistent communication ensures a great majority of issues are addressed speedily and appropriately. Care NOLA will create and maintain a dedicated communication link across all involved parties, including health care providers, to lower family stress, avoid crises and reduce expenses.
