We are seeking a compassionate, organized, and resident-focused Case Manager to join our team. The Case Manager plays a key role in coordinating resident services, supporting individualized care plans, and ensuring residents receive the highest-quality support in a safe, comforting, and enriching environment. This position also requires experience working with residents in Memory Care, as well as skills in community relations, admissions coordination, and guiding prospective residents and families through the onboarding process.
