We assist individuals with intellectual, developmental disabilities, autism spectrum disorder, brain injury, mental illness, co-occurring disorders, and physical challenges. The Mission of Community Transitional Center is to provide a safe and secure environment for individuals and their families to thrive in a person-centered thinking community that enhances and promotes positive control and self-direction through employment, education, and mental health rehabilitation. We assist individuals – with intellectual, developmental disabilities, autism spectrum disorder, brain injury, mental illness, co-occurring disorders, and physical challenges. Whether providing assistance at a challenging time or supporting an individual throughout their lifetime Community Transitional Center is there to discover, build and celebrate the strength in all people. Based on each individual’s level of need, our residential programs provides up to 24 hours of support each day. These supports may include rehabilitation, nursing, behavioral support and direct supervision by staff, depending on the unique needs of each individual we serve. In addition, we provide residential supports for individuals who have co-occurring mental health and substance abuse diagnoses. As with all of our residential programs, our goal is to integrate people with disabilities into the fabric of our communities. INDEPENDENT LIVING
For individuals who are able to live more independently, Community Transitional Center offers support in individual homes and apartments. Our supported independent living services are offered for as long and as often as needed, with the flexibility required to meet a person’s changing needs over time. Throughout our residential programs, we have the latest technology to enhance care and increase autonomy, empowering individuals to live fulfilling, independent lives.
