Dr Hazen’s father practiced dentistry from 1961 to 1998. Dr. Hazen joined his father’s practice in 1991 and continued upon his retirement in 1998. Dr. Hazen and his staff are committed to running a dental office that is focused on patient care. Each team member believes in providing patients with the same quality of care and personal service that they would give their own families. Mainland High School, Daytona Beach, Florida, graduated Magna Cum Laude in 1982
