"Bad Combination" — What would explain this patient’s retinal findings and report of sudden vision loss?

A 48-year-old woman presented for a routine examination with a chief complaint of blur at near. She explained she wanted to be able to read small print through her glasses “a little better.” Her ocular history was unremarkable. Her systemic history was positive for well-controlled hypertension. She was not diabetic. She denied trauma and allergies of any kind.

His best-corrected entering VAs were 20/100 OD and no light perception OS at distance and near. His external testing demonstrated normal ocular motilities OU. There was an afferent defect OS and no light perception upon confrontation field testing. Biomicroscopy demonstrated normal anterior segment structures with no neovascularization of the iris. IOP measured 15mm Hg in each eyes. The dilated fundus finding OS is demonstrated below.

🎃 Halloween 🧟‍♀️ Season Is Here! 👻

This is your yearly reminder that your Halloween costume is not more important than your sight. Halloween costume contact lenses are not safe.

You aren't looking at a spooky moon, that is a cornea the day after Halloween, or rather a missing cornea. The large green circle is the lack of epithelial tissue on the cornea after a patient wore a costume contact lens the night before. As a reminder, the cornea has more nerve endings than any other tissue in your body. A scratch on the cornea is one of the most painful things you can ever experience. The only way this photo was possible was with a significant number of numbing drops.

Contact lenses being sold without a prescription by a doctor are illegal in the United States, whether or not they have a refractive correction. Even contact lenses purely for cosmetic reasons require a prescription. If you see contact lenses being sold without a proper fitting by a doctor to ensure safety, report them to stopillegalCLs@aoa.org.

"Fading Fatigue" — Treatment strategies in myasthenia gravis aim to leverage mechanisms of autoimmunity.

A 56-year-old woman presented for follow-up one month after hospital discharge following evaluation for acute-onset complete right upper lid ptosis and vertical diplopia, which was initially noted when driving home after work. She reported full resolution of symptoms approximately two weeks after beginning pyridostigmine with titration to 300mg daily. She also reported no changes to vision, no muscle weakness, no change in voice or speech, stooping of the neck or difficulty swallowing or difficulty breathing. Best-corrected acuity was 20/20 OD and OS.
