Flourishing Communities One area of focus is the intersectionality of three key areas for individual and community well-being: food insecurity, mental health, and chronic diseases.

Flourishing Communities, Inc., is a faith-based nonprofit focused on improving the quality of life for people of color negatively impacted by the social determinants of health. Specifically, the organizational objective is to address the bio-social and spiritual components of human health with the goal of creating a trajectory toward wellness. A key focus of the ongoing work of Flourishing Communities is situated within the Larimer and adjacent communities which have suffered from persistent economic, social and health disparities for several decades.

📢 Allegheny County Health Department Announces New BOLD Coalition to Champion Brain Health 🧠💙

We’re excited to share that the Allegheny County Health Department is leading a new, collaborative effort to address the growing impact of dementia in our community.

Thanks to support from the CDC’s BOLD (Building Our Largest Dementia Infrastructure) grant, we’ve brought together 13 local partners to form the Allegheny County BOLD Coalition — a team dedicated to expanding education, increasing access to resources, and strengthening connections across care systems to support healthy aging and reduce dementia risk.

What we’re focusing on:
• 📘 Education for healthcare providers, caregivers, and community members
• 🧩 Outreach to at-risk communities and underserved populations
• 🤝 Connection between healthcare and supportive services across the county

“The answers are simple: know the signs, start the conversation, and connect to care,” says Dr. James Weeden.

We’re proud to work with partners countywide on this important mission — because better brain health starts with informed, connected communities. 💪
