Community Doula Alliance is a Black Woman-led, 501c3 nonprofit in Portland,Oregon that trains and supports doulas of color as they help families in their communities navigate birth & postpartum. Mission:
Our mission is to create an exceptional generation of professional doulas, particularly Black, Indigenous, Latino/a/x, Asian and other racially and linguistically diverse, underrepresented doulas,

who can enter the workforce with sustainable careers, improve maternal and infant health outcomes, and meet the ever-changing needs of birthing and postpartum people. Vision:
Our vision is to decolonize the birth work profession and eliminate the gap in maternal-infant health disparities that disproportionally impacts Black, Indigenous and Latino/a/x communities. Strategic Goals:
• Increase diversity in the doula profession by training Black, Indigenous & Latino/a/x doulas as curriculum trainers and facilitators
• Bridge the gap between doulas and the Traditional Health Worker registry process
• Prepare Black, Indigenous & Latino/a/x doulas to succeed in the workforce by providing business training, mentoring, and a supportive peer community
• Ensure that underrepresented doulas make a livable wage by advocating for the value and effectiveness of doula services
• Improve maternal-infant health outcomes, particularly within Black, Indigenous & Latino/a/x communities, by providing long-term, culturally-specific perinatal, labor support and postpartum doula care
• Increase equity awareness in the doula profession through education, training and community forums using a racial and economic justice framework
• Collaborate and build sustainable partnerships with community-based organizations, policy advocates, local maternity care hospitals/clinics and community care medicaid funding organizations

This is not a suggestion

Standards of Conduct can get a bad wrap for maintaining the colonized systems holding our communities back.

Especially in the doula profession.

But what if you considered them to be ethical pillars versus boxes you check for your certification renewal? 👀

When you start the professional pathway to be a THW, you'll have to understand the standards of conduct as the foundation of a trauma-informed approach to the work YOU do and the families you serve.

You are agreeing to:

💜 Represent all aspects of professional capabilities and services honestly and accurately

💜 Develop positive collaborative partnerships with community members, colleagues, and other health care providers to provide care, services, and supports that are safe, effective, and appropriate to a community member's needs

💜 Act as an advocate for community members and their needs

💜 Recognize and protect an individual's rights to dignity, respect, and freedoms from discrimination and neglect, to name a few

There's definitely more on the list for the standards because this is what it means to move from a culture of checked boxes in a system to upholding the dignity of your community.

The vital tool we need uplift Black birthing individuals💜

Created by , the Black Birthing Bill of Rights is a resource for every Black person that engages in maternity and perinatal care.

With over 30 rights, it includes:
✨Your right to choose your support team like a doula or midwife
✨Your right to advocacy
✨Your right to have your culture respected
✨Your right to to be believed and heard
✨Your right to anti-racist maternity care, and so many more!

It also serves as guidance for government programs, hospitals, maternity providers and others as they transform their policies, procedures, and practices to meet the needs of Black birthing people.

Let's talk about the other RBF...

Racial Battle Fatigue is where your body keeps the score of every microaggression you’ve ever survived.

Medical charts often list "hypertension" or "pre-term risk" as a biological inevitability for Black and Brown mothers. But they rarely talk about the actual weathering.

Weathering is the physical erosion of the body caused by constant exposure to systemic stress and Racial Battle Fatigue. It’s why a Black mother’s "biological age" can be 10 years older than her chronological age.

This isn't just a health stat. It’s a call to action to examine our "standard of care."

This is why culturally specific, community-based doula care is essential—it is a life-saving buffer. We don't just advocate in the delivery room; we help shield the nervous system from a system not built for us.

We need to stop blaming genetics for high-risk Black pregnancies. That's the easy answer. The real answer is much more expensive.
