☺️ Case managers are a crucial part in ensuring a smooth & safe discharge plan from the hospital to Persimmon Ridge. We are so lucky to work so many wonderful case managers & have had fun thanking them throughout the week!

✨ We have been updating our facility over the last few years, including updates to the exterior, new interior paint & artwork & a beautiful new therapy gym!
