An avulsion fracture of the fifth metatarsal occurs when a tendon pulls off a small bone fragment at the base of the fifth metatarsal, often due to ankle twisting or trauma during sports or falls. Symptoms include sudden outer foot pain, swelling, bruising, and difficulty walking. Diagnosis is confirmed with X-rays, with MRI/CT for complex cases. Treatment is typically non-surgical, involving RICE (rest, ice, compression, elevation), immobilization in a walking boot or cast for 4-8 weeks, and physical therapy. Surgery, using screws or plates, is reserved for displaced fractures or nonunion.
