Fall Prevention In-Service at The Reserve at First Colony

Fairmont Hospice was proud to partner with The Reserve at First Colony, Kindred Rehabilitation Services, and Fairmont Home Health for a collaborative Fall Prevention In-Service!
Falls remain one of the leading causes of injury among seniors, and prevention starts with teamwork and education. Together, we reviewed:
✔️ Common fall risk factors
✔️ Strength and mobility support
✔️ Environmental safety strategies
✔️ Medication considerations
✔️ When acute rehab, home health or hospice support may be appropriate

It’s powerful what happens when hospice, home health, rehab, and facility teams come together with one shared goal — keeping residents safe, strong, and supported.

Hospice Facts Wednesday

Hospice isn’t about giving up… it’s about living well. Here are a few facts that might surprise you:

1. Hospice is about quality of life.
Hospice focuses on comfort, dignity, and support — physically, emotionally, and spiritually.

2. Hospice comes to YOU.
Most hospice care happens wherever the patient calls home — private residence, assisted living, or skilled nursing.

3. Hospice is covered by Medicare.
For eligible patients, hospice is typically covered 100% under the Medicare Hospice Benefit.

4. It’s a full team approach.
Patients receive care from nurses, aides, social workers, chaplains, physicians, and volunteers.

5. Families receive support too.
Hospice provides education, emotional support, and even bereavement services for loved ones.

6. It’s not just for the last few days.
Many families say they wish they had started hospice sooner. The earlier the support, the greater the benefit.
