MY DOCTORS OFFICE FAXED IT TO THEM WHILE I WAS ON THE PHONE WITH THEM. THEY CANCELLED MY APT AFTER TELLING ME TO CALL MY DOCTOR AGAIN FOR A REFERALL. DONOT WASTE YOUR TIME WITH THESE IDIOTS.
