🏡 We visited our Recuperative Care home
This home is used as Recuperative Care — a short-term, supportive housing setting for individuals who have been discharged from the hospital but do not have a safe place to recover from illness or injury.
Recuperative Care exists to ensure that people can heal in a safe, stable, and dignified environment, instead of returning to the streets or unsafe living conditions.
What Recuperative Care provides:
✔️ A bed and temporary housing
✔️ Meals
✔️ Monitoring of medical and behavioral health conditions
✔️ Transportation to medical appointments
✔️ Case management
✔️ Connections to housing and supportive services
Eligibility criteria:
An individual must meet both of the following:
1. Medical need:
• Requires recovery from illness or injury
• Has a condition serious enough to risk ER use or hospitalization
• Safe recovery is not possible in their current living situation
2. Housing instability:
• Currently experiencing homelessness
or
• At risk of losing housing

📌 Important (Compliance Notice):
Recuperative Care services are available only to eligible Medi-Cal members.
Participation requires meeting all eligibility criteria and
authorization through the member’s Managed Care Plan (MCP).
Service availability varies by MCP and county.
