Meet our Chief of Clinical Services, Jordan......and also wish her a HAPPY BIRTHDAY!!!!

Jordan’s nursing journey began at the bedside, where she worked as a CNA in long-term care and later in Med-Surg at Lafayette Regional Health Center—a rural hospital serving a predominantly geriatric population with complex chronic conditions. While continuing to work full-time, Jordan earned her LPN and then her RN, building a strong foundation rooted in patient-centered, hands-on care.

After becoming an RN, Jordan spent five years in hospice nursing, including three years as an RN Care Manager. In the final two years of her hospice career, she stepped into leadership roles as Assistant Director of Nursing and later Director of Nursing. It was during this period that her passion for advocating for underserved and vulnerable populations truly took shape, as she worked to ensure patients received the dignity, care, and attention they deserved.

For the past two years, Jordan has been a valued part of SparroWell, a company founded on Ernestina’s deep passion for the post-acute care space. That mission resonated deeply with her and continues to drive her work each day. She currently focuses on the Clinical Operations, focusing on supporting clinical teams, strengthening partnerships, and ensuring high-quality, compassionate care across the communities SparroWell serves.

Meet Our Founder

Ernestina Adkins
Founder & CEO, SparroWell

Ernestina Adkins is a nurse with over 17 years of experience dedicated to improving care for older adults across the long-term and post-acute care continuum. She is the sole founder of SparroWell, a company built from firsthand clinical experience and a deep commitment to restoring connection and consistency in a fragmented care system.

Her journey began at the bedside as a Certified Nursing Assistant (CNA) while attending nursing school. After graduating nursing school in 2009, Ernestina advanced through progressive clinical leadership roles within the Post-Acute Care sector - serving as a Charge Nurse, Director of Clinical Education, the Director of Nursing, followed by United Healthcare. In these roles, she led staff training, regulatory preparedness, clinical oversight, and survey readiness—ensuring teams were supported, confident, and compliant.

After five years in facility leadership, Ernestina moved into a Regional Clinical role with a large nationwide assisted living organization. There, she specialized in facility turnaround, working closely with teams across Missouri and Iowa to strengthen compliance, improve survey outcomes, and implement sustainable policies and procedures. This work reinforced her belief that strong systems matter—but only when they are built around people.

Seeking broader, system-level impact, Ernestina joined UnitedHealthcare / Optum as a Senior Analyst in the post-acute care space, gaining critical insight into value-based care, payer strategy, and the operational gaps facing long-term care providers nationwide.

In 2021, Ernestina founded Care Midwest, initially envisioning a small regional presence. That vision quickly expanded. Today, operating as SparroWell, the organization supports partners nationwide, delivering clinical expertise, operational rigor, and scalable solutions rooted in compassion and trust.

SparroWell was founded on a simple but powerful belief: It Starts with You.
The nurse who calls a patient on their hardest day.
The coordinator who sees the whole person.
The provider who chooses presence over protocol.
The teammate who shows up with empathy, intention, and heart.

At her core, Ernestina remains a nurse first—and she openly embraces the ongoing journey of learning how to lead and grow a company. A new space that reflects both our growth and our heart.

Our new SparroWell office represents intentional growth — a place designed to bring together nurses and caregivers who believe deeply in the work we do. This team comes together every day to support our partner practices and improve the lives of seniors, driven by compassion, commitment, and purpose.

What began through word of mouth has grown into SparroWell supporting communities in 35 states, built on trust, relationships, and meaningful outcomes. Yet at the center of that growth is what has never changed — people who care, and work that truly matters.

We’re proud of this team, grateful for how far we’ve come, and excited for what’s ahead.

Over the last two months….SparroWell has traveled to Oklahoma, Minnesota, DC, and Florida.

Despite how it looks…..we were actually working 12-16 hour days in our hotel rooms! We will neither confirm nor deny eating entire jugs of ice cream from the bed while we worked!
