Dear Editor,

Thank you for highlighting the overreliance on continuous electronic fetal monitoring (EFM) and its role in rising Cesarean rates.

There is a solution to this epidemic. Midwifery-led birth centers use intermittent fetal monitoring instead of continuous EFM. They are an evidence-based model that delivers outcomes comparable to those of the top nations globally by offering holistic, family-centered care that protects and empowers patients, thereby enhancing both physical and mental well-being in our community.

Birth centers are fully integrated into the healthcare system and guided by principles of prevention, safety, appropriate medical intervention, and cost-effectiveness. While midwifery care can occur elsewhere, birth centers are unique in offering this dedicated, physiologic model.
Outcomes speak for themselves:
-Cesarean rates under 6%
-NICU admissions less than half the national average
-Low rates of interventions with superior overall outcomes

These results demonstrate the clinical excellence and fiscal prudence of the birth center model, reducing complications and costs while maintaining world-standard safety, lowering maternal mortality risk, and prioritizing maternal autonomy and satisfaction.

Is your baby in a posterior position? here are some Spinning Babies techniques to help encourage baby to rotate over to an anterior position.
⭐️Forward-Leaning Inversion
Purpose: Creates space in the lower uterus and helps baby move out of the pelvis temporarily to reposition.
How: Kneel on the edge of a couch or bed. Slowly lower your hands to the floor so your hips are higher than your head. Keep your arms, head, and neck relaxed. Hold for 3 slow breathes, then come back up slowly
Tip: Try once when you wake up and again before bed. Follow with pelvic tilts or brisk walking. *Avoid if you have high blood pressure, dizziness, or reflux.
⭐️Side-Lying Release
Purpose: Releases tension in the pelvic muscles and ligaments, allowing baby to rotate.
How: Lie on a firm surface on your left side. Bottom leg bent, top leg hanging off the edge, relaxed. Have someone stabilize your hip to keep alignment. Relax in this position for 3–5 minutes. Repeat on the other side.
⭐️Modified Cat-Cow
Purpose: Gently moves the pelvis to make space and encourage baby to shift.
How: Get on hands and knees in tabletop position. Inhale and gently arch your back upward like a cat, tucking the tailbone. Exhale and return to a neutral, straight back (do not drop the belly low). Repeat for 10–20 slow reps.
⭐️Pelvic Tilts
Purpose: Mobilize pelvis and encourage baby to drop into a better position.
How: Sit on the ball with feet flat on the floor. Do gentle pelvic circles, figure 8s, and rocking. Spend 10–15 minutes per session.
⭐️HIP DIPS
Exaggerated figure 8 movements to make room in the pelvis for the baby to rotate from the right to the left, and from posterior to anterior (by reducing tightness in back preventing baby’s ability to rotate).
How: Lean over counter with legs somewhat straight but relaxed and back at a flat 90 degree angle. Lift one hip while lowering the other. https://www.facebook.com/reel/1556123808396868
