This virtual training equips community health workers with the tools to support individuals living with Crohn’s Disease and Ulcerative Colitis by helping them navigate care, access resources, and advocate for better health outcomes.

The IBD Patient Navigator® provides individualized navigation, education, and advocacy support to patients diagnosed with Inflammatory Bowel Disease over a six-month period, with a focused on improving health outcomes and continuity of care.

Navigators will work directly with patients to strengthen IBD disease management and support measurable behavior changes that lead to:
1. Reducing ER and hospital visits by 10%.

2. Increase confidence navigating the healthcare system.

3. Implement Dietary Changes at home.

4. Increase social engagement.
