Every year I look forward to publishing our stats, namely transfer stats.
This year the two surprising and rare emergent transfers in labor were for cord prolapse & non-reassuring fetal heart tones. Both women had histories of normal homebirths. Both had happy outcomes. This is what you hire us for.
While around 1/3 of our population has elevating normal blood pressures in late pregnancy, we had 2 risk out this year ahead of active labor for wandering into PreEclampsia territory. Our regs and care for safe outcomes do not permit us to continue on with these cases. One woman risked out for breech at term, electing surgical birth. Some women with histories of PreE return to us in future pregnancies with a resolve to overhaul nutrition & lifestyle to prevent adverse health and birth outcomes and we have pulled off future births with no issues. This is a pesky clinical picture with 1-2 women risking out annually for metabolic dysfunction in the practice.
We had 5 (not 4) first time birthers elect hospital transfer in early or non-active labor for clinical exhaustion, pain management, labor augmentation by docs. This was typically in the 0-5cm space over 12-30 hours of labor which is considered ‘early’ labor. As midwives we want to be sure we are preparing women prenatally for the mental and physical demands of labor and eduction on the normal phases of labor to reduce these numbers. Compared to the North American stat on this from a BMJ study in 2005 that suggested 1:4 (25%) first time women planning OOH birth will wind up in hospital anyway, we are doing very well at 10% with this stat. Experienced professional Doulas, childbirth education courses, self education & RADICAL PRIVACY tend to keep first time birthers out of this territory.
We are grateful for a fun and incredible year with most of our client base elated with their experiences and turning into life long friendships. We sacrifice normal lives to take call and offer this work. We are in it for the women and the transformations. And for what we offer outside institutional maternity, for women who choose to show up for themselves. 🎊🥂🌸♥️
