In 2022, the World Health Organization reported that obesity and overweight, which are strong independent risk factors for MASLD, affect almost 60% of adults and nearly one in three children in Europe. With its increasing prevalence rate, MASLD is becoming the most common indication for liver transplantation and the main underlying aetiology for the development of liver cancer.

[US] imaging, the standard method of surveillance for , has a sensitivity of only about 75%, falling to 50% in small cancers. In addition, it’s operator dependent. Alpha-fetoprotein [AFP] has been proposed as a marker for , but it is not enough as a stand-alone marker.

This study shows that a one-time evaluation of sound waves from the portal vein via Doppler US using AI can detect small HCCs with greater than 80% accuracy, and the combination of AFP and portal vein analysis can detect small HCCs with an accuracy of more than 95%

“For patients with alcohol-related liver disease in earlier stages, we see a high progression rate — 15% progressed to decompensation,” Johansen said. “We also see high mortality rates, with 101 deaths within 6 years. Alcohol intake and fibrosis stage were the strongest predictors of events.”

"…we need to get SAFE into primary care so these doctors can automatically calculate their scores, and if the patient is over 100 [high risk of chronic liver disease], then they need help [referral to a hepatologist]."

Ray Kim, MD of Stanford University School Medicine, presents an algorithm, the Steatosis-Associated Fibrosis Estimator (SAFE), developed to detect clinically significant fibrosis in patients with nonalcoholic fatty liver disease (MASLD formarly known as NAFLD). It is effective at detecting chronic liver disease from all causes with or without [MASLD] in the general population, according the results of a US population-based study. The algorithm was designed for use in primary care to help slow the steep rise in liver disease burden. “Primary care doctors need to engage with the issue and have appropriate tools to identify people with liver disease."

The SAFE score can be used to detect chronic liver disease regardless of steatosis in the general population, offering the possibility for earlier detection in primary care.
