Achilles pain ≠ always an Achilles tendon problem 🦵
Many people are told they have “Achilles tendonitis.”
But when we look with musculoskeletal ultrasound 🔬, the story is sometimes very different.
In this scan of the Achilles insertion, the tendon fibers themselves look fairly healthy. But notice the color Doppler signal lighting up just in front of the tendon near its attachment on the heel bone 👀.
That increased blood flow can indicate irritation in the deeper interface tissues near the insertion, not necessarily damage within the tendon itself.
Why does this matter?
Because if we assume the tendon is the problem, treatment may miss the real pain generator. The right diagnosis helps guide the right loading strategy, mobility work, and recovery plan to actually solve the issue.
