By acting as a compassionate mediator, we help overwhelmed families take the emotion out of difficult decisions to focus on finding the best care for their loved ones. We aim to make the process as stress-free as possible by providing the education and clarity needed during a challenging time.

Assisted Living: A social, community-based setting perfect for those in the earlier stages of a diagnosis who enjoy activities but are still safe on their own for a few hours.

Adult Family Homes: A smaller, residential option for those needing a higher level of care. It’s a "homey" environment with less stimulation—ideal for anyone who gets overwhelmed easily.

Memory Care: A secure community specifically designed for safety. While not every dementia diagnosis requires it, it’s the right choice if a loved one is prone to wandering or being unsafe.

Every family that we work with, we have a packet that we give you.

It really goes into detail what our three-step process is, so that if you have any questions along the way—like when we’re not meeting together—you can always refer to that.

And we’re always available by phone, text, email to answer any of your questions.

We want this to be a smooth process, and it can be really stressful and you can feel anxious about kind of the unknown, and we just, we don’t want you to feel that way.

And so anyway we can help, even if it’s just answering a question, we’re there for you.

When families call us, there really isn't a lot they need to have prepared before we meet for a care consultation.

Really just an openness. An understanding that their loved one needs care and that we are here to help.

We are here to guide you through this. To help you navigate it. To create a roadmap that gets your loved one into the care they actually need.

And when it comes to selecting a community, all of them are going to provide similar levels of service. What it really comes down to is the vibe. Who your loved one is going to connect with. Where they are going to fit in best.

You are going to feel it when you are there.

We have a 3-step process when we work with families, from the first call all the way when your loved one gets moved in.

We start with a care consultation to understand your loved one — their care needs, their medical diagnosis, their likes, dislikes, their personality — so we can determine what the best setting is going to be for them. Then we go on tours together so nothing gets missed. And when it's time to move in, we help with everything — movers, doctor's orders, whatever it takes.

Hi, I’m Jacqueline with Assured Care Placement Services, and we offer a no-cost service.

We help families when they have a loved one who can’t live safely at home anymore. For example, if mom falls, breaks a hip, or just has any kind of decline in their health, we’re able to get them connected to the best care community, whether that's an adult family home, memory care, or whatever is going to suit them best.

My number one question to always ask — what is the staffing to resident ratio, and how many of those are actually caregivers?

Because communities will tell you the ratio looks great. What they won't tell you is they're counting people who won't be answering a call button.

You need to know who is actually showing up when your loved one needs help. And how long it takes them to get there.

One of my favorite stories, there was a lady that we place into an adult family home that had cancer. I went to visit about a week, maybe two after she moved in and I always bring a little gift.

When I got there she was all smiles, because what I didn't know is she had gone into the doctor, and learned that her tumor had shrunk significantly. What I really love about what I get to do is being able to visit and to celebrate those wins with them.

A lot of the time people come to us in crisis, they've lost mobility or something like that and then being able to see that progress and to see them thriving. And sometimes — not always, but sometimes — you get to watch them turn a corner. And when that happens, you celebrate it with them like it happened to you.

Myself and a provider at an adult family home. She specializes in dementia care and has helped her residents heal after injuries and health conditions like strokes. She's absolutely amazing and they provide the best care.

We have an extensive list of questions that we ask in our care consultations so we can get a full picture of what your loved one needs. Some of those questions cover different behaviors they might have.

At Assured Care, our goal is to get the full picture from the start so we can find a home where they can stay comfortable and settled for the long term.
