There are certain moments when having a licensed physical or occupational therapist come directy to the home makes all the difference.

Here's when you would use Smart Rehab for yourself or a loved one:

- after being discharged from home health, but you still haven't reached your fullest potential
- a recent fall or a noticeable increase in unsteadiness
- general weakness or difficulty getting out of a chair
- decline in walking ability or needing more assistance than usual
- after surgery, (hip, knee, back, shoulder) when getting to a clinic is tough
- new pain that's limiting daily function
- worsening balance, fear of falling, or loss of confidence
- trouble getting to outpatient therapy 2 times per week
- a need for home safety check or equipment recommendations
- condition's like Parkinson's or stroke where consistent, one-on-one therapy at home is more effective.

Smart Rehab brings outpatient level therapy directly into the home, with 60 minutes of one-on-one care from a licensed therapist at every visit, and no homebound requirement.

Smart Rehab is perfect for older adults who prefer the convenience and comfort of therapy in their own home.

We are not home health, so patients do not need to be homebound.

Ideal patients include older adults dealing with:

- difficulty walking or unsteady gait
- falls or near falls
- pain limiting daily function
- declining strength or endurance
- post surgical recovery (hip/kneee/back/shoulder)
- Parkinson's, stroke, or other neurological conditions
- need for a home safety or equipment assessment

If getting to a clinic is tough, or they want more focused, one-on-one time, we bring the care to them.

Meet Jill Vonderhaar Nader, PT, our Clinical Market Director for Northern Kentucky!

Our expansion into NKY started with Jill, a longtime friend and Saint Louis University grad, who thought Smart Rehab would be a perfect fit for her hometown community.

Jill Vonderhaar Nader and Theresa Schuh Finlay actually went to high school together, and Jill was one of the reasons Theresa chose Saint Louis University Program in Physical Therapy for PT school! Now, years later, they're teaming up again to bring high quality, one-on-one therapy services to the homes of older adults in Northern Kentucky.

As our Clinical Market Director for NKY, Jill will continue treating patients while helping to build a team of compassionate, skilled therapists and strong relationships with local referral partners who share her commitment to exceptional care.

We are excited to announce Smart Rehab is now providing in-home physical and occupational therapy services in Northern Kentucky, part of the greater Cincinnati area!

This expansion is especially meaningful for us as Theresa Schuh Finlay, PT, co-founder of Smart Rehab, grew up in Northern Kentucky, and we previously lived and worked in this community. Bringing our mission to Northern Kentucky of helping older adults stay strong, independent, and home feels like a true homecoming!

We are also thrilled to welcome Jill Vonderhaar Nader, PT, as our Clinical Market Director for Northern Kentucky! Jill will lead our efforts in promoting Smart Rehab's in-home therapy services and in building a team of experienced and compassionate therapists who share our commitment to providing exceptional care for older adults.
