A recent incident making headlines highlights something we talk about all the time in our industry: willful safety violations.

In this case, investigators found that critical safety requirements were not just missed—they were knowingly disregarded, ultimately contributing to a fatal outcome. 

Let’s be clear about what that means.

A willful violation isn’t a mistake.
It’s when an employer knows the standard, understands the hazard, and chooses not to comply anyway—or shows plain indifference to worker safety. 

This is the most severe classification OSHA can issue—and for good reason.

Because behind every willful violation is usually:
• A known hazard that wasn’t corrected
• A shortcut taken to save time or money
• A breakdown in leadership, accountability, or safety culture

And too often… the consequences are permanent.
