Choices for Change Counseling Agency and WMed Street Medicine have entered into a new, collaborative mission that combines street medicine and case management, as well as addictions treatment, psychiatric interventions, crisis response, cognitive-behavioral therapy, and reality therapy. The clinic and medical organization will be jointly working with clients in their homes, motels, tents, or wherever the patients/clients are located.

The teams will encompass doctors, social workers, counselors, student doctors, interns, masters'-level counselors and social workers engaging in ACTIVE (not only reasonable) EFFORTS at the micro-level to connect with clients/patients to build trusting relationships.

This approach takes into account transportation barriers faced by so many in the community. We fill the transportation gaps and accompany clients/patients if so desire. We started outreach care this week. We are so excited about our new joint venture.
