CCMC earned a prestigious 5-star rating for excellence in helping patients confidently continue their recovery after discharge. From understanding medications to coordinating follow-up care. This recognition was featured in Becker’s Hospital Review’s list of “204 Top Hospitals for Care Transitions.”
