Wake County (NC) is looking to reduce the use of lights and sirens. The goal is to reserve lights and sirens for only the most critical emergencies, such as shootings, stabbings, or severe vehicle crashes. Currently, about 80 percent of EMS calls use lights and sirens; under the proposed policy, that would drop to 20 percent.

Treat in Place (TIP) option for Onslow County (NC). Bradley Kinlaw, Onslow County Emergency Services director, said the changes will make EMS response more agile and focused. “Not every medical call needs an ambulance,” Kinlaw said. “Now when someone calls 911 for something like lift assistance or other minor issues, we can send a community paramedic, which ensures that ambulances are available for the most serious calls.”

The economic model for transformations like this are a bit challenging. The Q2 PWW|AG Financial Index revealed that nationally, 39% of ambulance Treatment in Place (TIP) claims are paid, with an average reimbursement per paid claim of $398.

Although it is difficult prehospital to elevate IV bags a full meter above the patient’s IV catheter, doing so substantially increases IV fluid flow, regardless of catheter size. Pressure bags and pumps do make this less of an issue.

In a study looking at IV flow rates through peripheral IV catheters, increasing the height of a 0.9% NS IV bag or even plasma, increased flow through an 18G IV from 40 ml per minute at 1 foot to 86 ml / minute at 2 feet, and 136 ml / minute at a little over 3 feet of elevation.

Similar relative improvements in flow occurred with all sizes of catheters.

From Maia Dorsett: Only 54.6% had a complete set of vital signs documented, while 9.8% had no documented vitals, but I think the money is in the figure below - the dispatch codes, associated incidence and vital sign performance by patient age group.

Pay attention to "Falls". Calls dispatched as a "fall" for patients age 60 and older accounted for 15.1% of all non-transport incidents with incomplete vital signs and 18.2% of patients with no vital signs.

Given the documented morbidity and mortality associated with "lift assist", this paper quantifies the sweeping lack of assessment of these medically complex patients that we encounter every day.
