Did you know you can now request your official vaccine records directly through the Kansas Department of Health and Environment (KDHE)?
Kansas maintains a secure vaccine registry called KSWebIZ, which stores records for:
Vaccines given anywhere in Kansas
Vaccines given in other states, if your provider reported them to the registry
Through the KDHE WEBIZ patient portal, you can quickly request and download your official immunization record without needing to contact your provider’s office. This makes it easy to access the information you need for school, travel, work, or personal records.
Important: Before using the portal, please make sure your provider has your current cell phone number and personal email address on file. This information is required to verify your identity and complete your request. Without it, you may not be able to access your record online.
