Get ready! Medicare Audits are coming, make sure you have READY the following:
The ADR letter will contain the following information:

Preliminary dispensing order (if items were dispensed prior to obtaining a detailed written order);
A valid order;
The patient’s medical records that may support the medical necessity for the items provided and the frequency of testing ordered;
Documentation from the physician’s records OR the beneficiary’s records that support/verifies the frequency with the beneficiary is testing his/her blood glucose level;
Itemized proof of delivery documentation
Proof of beneficiary’s or caregiver’s request for refill
Any other pertinent documentation
Advance Beneficiary Notice (ABN) on file should

The beneficiary's state of residence on file with the Social Security Administration determines the DME MAC jurisdiction that should be billed. A claim should be sent to the DME MAC for the state in which the beneficiary permanently resides.
