The metal framework is attached to mini-implants (TADs) placed in the palate.
A central screw is activated gradually.
When turned, it widens the maxilla (upper jaw).
This is different from a regular tooth-borne RPE because: 👉 Force is applied more to bone, not just teeth
👉 Better skeletal expansion
👉 Less dental tipping
🦷 Indications in an 8-Year-Old
At 8 years, we usually consider:
Narrow upper arch
Posterior crossbite
Crowding
Mouth breathing / airway concerns
High-arched palate
However, true MARPE is more commonly used in adolescents and adults when the mid-palatal suture is more mature.
In an 8-year-old, many orthodontists would prefer:
Conventional Rapid Palatal Expander (RPE)
Or bonded Hyrax expander
Because sutures are still relatively open at that age.
⚠️ Important Clinical Considerations
For early skeletal expansion:
CBCT evaluation is ideal
Check midpalatal suture maturity (Angelieri classification)
Assess nasal airway benefits
Monitor root proximity
