Low-cost, synthetic oils may be okay for air fresheners and bath soaps, but we demand “food grade” raw material from whole plants for all Nápreva products. Dietary supplement standards accept up to 10% variation between labels and real product potency because terpene content in plants can vary widely from harvest to harvest. We blend and tests every batch to achieve no more than a 5% variation in terpene potency – usually less. Many of our customers suffer from lifelong conditions. Lots of people ask if Nápreva is FDA approved. It is not. It’s is a dietary supplement and there are no FDA-approved dietary supplements. But here’s something many people don’t realize: many medications are approved for one condition but commonly prescribed “off label” for pain — uses the FDA has not reviewed or approved. Antidepressants, seizure medications, muscle relaxants, and sedatives are often used this way. Some help. Some don’t. Some come with serious risks when used long term.

In a landmark 2004 law suit, the maker of Neurontin (gabapentin) was found guilty of marketing the product for a wide range of unapproved uses. It's only FDA-approved for seizures and post-herpetic neuralgia, but it was widely marketed and prescribed off-label for pain and psychiatric conditions without solid evidence. The case was settled in two phases for a total of $755 million.

So the most important question isn’t just whether a drug is FDA approved, but is it approved for long-term pain management. The answer in many cases is no. And in an equal number of cases, long-term side effects are far more dangerous than any experience you may have with dietary supplements like glucosamine chondroitin, omega 3 fatty acids, turmeric or Nápreva.

We have a lot to learn about chronic pain and how to treat it. In the most comprehensive analysis of studies on the use of tramadol for treating chronic pain, it's been found that it has little effect on relieving pain, but has significant and damaging side effects.

It's unfortunate that with conditions as widespread as chronic pain it can take years, even decades of field use to determine true efficacy and get quantifiable data on side effects.

Not all drugs are evil. But with complex conditions like chronic pain, there's no one-size-fits-all solution. Let's hope that all healthcare professionals -- traditional, ancient and modern -- find ways to work together toward better setting expectations and helping people to craft personalized solutions that truly suit their needs.
