Clinical Bridges We are a transitional company helping patients transition from an acute rehab or hospital to home. We are covered by Medicare and other insurances.

We are not home health and we are not trying to replace your primary care physician, but we work with them to make sure that our patients have the utmost care. Services we provide:
Our nurse practitioners provide in-home visits for post-acute Transitional Care, chronic care, and palliative care. We are on call after hours and weekends and can act as PCP or coordinate with your current PCP and specialists.


• Patient phone contact within 48hr of DC

• NP visit at their residence with 7 days of discharge

• Refill and adjust
prescriptions

• Risk assessment at initial visit to determine risk of readmission and frequency of follow up visits

• Coordination of care and communication with home health, PCP, specialists.

• Thorough review of hospital and facility records with follow up on labs/diagnostics/orders. Records left for PCP f/u appt.

• Additional visits only as clinically indicated to prevent readmissions based on risk assessment.

• Education of the patient and family on the diagnoses, disease processes, medications, follow up appointments, and discharge instructions

• Assistance with coordinating readmission to the acute care/rehab system if indicated with 30 days DC

• Referrals of patients from the home to our partner facilities

Very proud to work with Shaun Carlton, Liberty Hill, Kehila Gamez, and Michelle Folds. They are all compassionate and so hard-working.

Our patients and partners hopefully know how lucky they are to have this great team caring for them.

Clinical bridges is growing! We are seeking 1 or 2 full time nurse practitioners right now one for Fort Worth and 1 for the Southeast area of dfw. We are also seeking an LVN to work from home doing patient triaging and other tasks.

Decided to use some of the stimulus money that was given to Clinical Bridges by the government to do a kind gesture for the 39 patients in 7 group homes for whom we are the P*P. Add fun shopping with my son.

I know they have been unable to go to their activity center daily Monday through Friday like usual due to the virus, and are probably bored to death stuck in their homes! Thought some fun stuff and snacks might cheer them up! Can't wait to deliver them!

Wanted to thank our scheduler Robin Robertson for being our cheerleader and talking up what our company does, and why we do it for over 25 minutes with a patient Friday!

I know her job talking to patients is tough somedays (ok most days!) and not everyone gets or appreciates it, but she tries so hard to help them get it! And I appreciate her!
