Advocates for the Aging, LLC Advocates for the Aging offers Geriatric Care Management to clients across NYS. We develop a plan of care to address the unique needs of each client and family.

Our services include, but are not limited to:

Coordination and Monitoring of Home Care Services

​Monitoring the Status of Loved Ones in Nursing or Assisted-Living Facilities, and Facilitating Ongoing Communication Among All Involved Parties

Assistance with Coordinating Medical Appointments, Including Arranging Transportation if Needed

Benefits and Entitlements Counseling, Including Medicaid Applications

​Arranging for the Services of Estate Planners and Financial Advisors as Needed

​Acting as a Liaison Among Clients, Families, Medical Practitioners, etc.

​Working With Social Workers and Discharge Planners to Safely Transfer Clients Back Into the Community Following a Hospitalization or Stay in a Rehabilitation Center
