This week our Blog focuses on the differences between Emergency Departments. Not all ED’s are created equal. There are differences between staffing, speciality care and resources. Plan ahead and know where your nearest emergency department is as you head outdoors or find yourself traveling. Knowing and advocating for the right emergency department may just save your life. Make a caricature based on what you know about me

I’ve been doing Emergency Medicine for 20 years. Seen a lot, learned even more. One thing I realized years ago, people are generally unprepared. Knowing this I set out to do something about it. September of 2025 PrepEM Wild started. What took years to build up to, convince myself I could be a business owner. Best decision ever was to just start. I’m only a Doctor, what do I know about starting or running a business? Doesn’t matter, just get it done. Thank you to everyone who has supported and encouraged me to keep going. This year is going to be fun. Refill kits and Hemorrhage kits coming very soon. Stay tuned, lots to do this year

Hypothermia doesn’t always look dramatic.
It looks like confusion. Poor decisions. Slow movement. Silence.

During severe winter storms, hypothermia remains one of the leading causes of preventable death in cold environments—and it doesn’t require subzero temperatures.

We just published a physician-written breakdown covering:
• How hypothermia actually presents
• Early signs most people miss
• Field management when help is delayed
• Prevention that works in real conditions

If you spend time outdoors, travel rural roads, or recreate in winter—this matters.

Frostnip vs Frostbite: The Color Change That Matters ❄️

Winter storms across the east coast so know the risk.

Here’s the key distinction people miss:

❄️ Frostnip (reversible)
• During exposure: redness, pale, tingling skin
• After rewarming: red, painful, sunburn-like appearance
• This redness is normal — it’s blood flow returning
• No permanent tissue damage

🧊 Frostbite (true emergency)
• During exposure: pale, white, gray, or waxy skin
• After rewarming: skin may stay pale OR turn blue, purple, mottled, and may blister within hours
• Indicates cell death and vascular injury
• Risk of infection, tissue loss, and amputation

• ~3,000 frostbite hospitalizations occur annually in the U.S.
• Up to 20% of severe cases lead to amputation
• Wind chills below –15°F (–26°C) can cause frostbite in as little as 10 minutes but typically 30 minutes give or take.

Field Management

Suspected frostnip
• Get out of the cold immediately
• Remove wet clothing
• Rewarm with skin-to-skin contact or warm hands
• Do NOT rub or massage

Suspected frostbite
• Stop cold exposure
• Rewarm only if refreezing is impossible
• Warm water 98–104°F (37–40°C) for 15–30 minutes if available
• Do NOT rub, pop blisters, or use dry heat/fire
• Protect tissue, splint if needed, and evacuate

🚫 Refreezing after thawing causes catastrophic tissue damage

The takeaway

Red and painful after rewarming = frostnip.
Pale, blue, purple, or blistering = frostbite until proven otherwise.

Numb doesn’t mean safe. Recognition and correct field decisions save fingers, toes, and lives.

Stay prepared. Stay wild.

Lower back pain is common — but sometimes it’s a warning, not an inconvenience.

Most back pain resolves with time.
But when pain is paired with numbness, weakness, or bladder/bowel changes, it can signal a true neurologic emergency like cauda equina syndrome.

In remote terrain, recognizing the difference matters.
There’s a point where rest and pain meds are no longer the answer — evacuation is.

We break down:
• When back pain is benign
• The red flags you should never ignore
• Why early action can prevent permanent injury
