Life Matters offers 2 types of services, personalized Geriatric Care Management and End of Life Doula Services.
Geriatric Care Management is a holistic, client-centered approach to caring for older adults facing ongoing health challenges. Comprehensive nursing experience and professional assistance helps guide you during times of uncertainty. This allows families to find solutions that ensure quality care and an optimal life for those they love, thus reducing worry, confusion, & stress for family members and caregivers. This is done through:
- Assessment of individual & caregiver needs
- Problem Solving, Planning, and Implementation of Care Plan
- Disease Process Education and Patient Advocacy
- On-going Monitoring of the Care Plan as needed
End of Life Doula service is also available through an individual's end of life journey. This is a unique way of ministering to people during the last stage of life in a way that achieves deeper meaning and offers greater comfort. Doulas view the dying process as significant and sacred as the birth process. Emotional, spiritual, and physical support is provided at an intensely personal and crucial time. Doula work may involve exploring the individual life's beautiful moments, finding meaning, creating a legacy project, planning for how the last days will unfold, and being present during the last vigil. The goal of this work is to empower an individual to finish their earthly journey in a way that embodies the best of who they are and inspires those they leave behind. The doula also offers support for loved ones during this time, and helps to ease the suffering of grief in its early stages.
