Crowe type IV developmental dysplasia of the hip is not a common problem we see these days. In modern day medicine, hip dislocation is caught usually at the time of birth and babies can be treated no surgically with braving. Unfortunately when the problem is bilateral, it’s harder to identify and this patient has been suffering from bilateral severe developmental dysplasia with secondary osteoarthritis her whole life. Basically her hips have been dislocated her whole life and have developed a new fake or “pseudo” socket. The arthritis got so bad she couldn’t handle it anymore and we planned to perform a complex reconstruction through a hip replacement. Hip replacement helps reconstruct the hip biomechanics in these patients however restoring the true socket or acetabulum may lead to TOO much limb lengthening and can cause damage to the sciatic nerve. To address this, I planned a shortening femoral osteotomy in conjunction with a total hip arthroplasty. To ensure the nerve was ok during surgery, I used SSEP neurological monitoring which ensured her sciatic nerve would not be overstretched. Surgery went well and she can’t wait to get the other side done. Check out the 2010 JBJS study out of the Mayo Clinic breaking down the surgical technique for this complex procedure. What to do with acetabular bone loss?

Certain patients with a significant amount of bone loss requiring a revision total hip replacements can require extensive planning to prepare for surgery. Sometimes patients unfortunately lose bone in their pelvis for a variety of reasons related to their disease or their prior surgeries. When enough bone is lost, the hip can fail and become unstable.

With modern day technology, we can now reconstruct a patients hip with patient specific custom components made to match their individual anatomy through CT scan imaging. This case was a custom triflange prosthesis made for a patient with severe bone loss in the pelvis (particularly the anterior column and wall). A custom triflange prosthesis was made with Biomet Zimmer. One of the many benefits of the custom prosthesis is that it fits into the patients anatomy and has inherent stability. Additionally, you can plan your fixation and screw placement all ahead of time based on CT imaging and planning. It was great working with Dr Daniel Allison and his team on this case! .md

Follow up post: Osteopetrosis, is a very rare condition that causes overly dense bone throughout the body. It has also been known as ‘marble bone disease’. The osteoclasts, which are the cells responsible for normal bone remodeling are defective and cause the bone to be super dense. Problem is when the bone gets so dense, it actually becomes brittle and then can break more easily. This lady had a failed right total hip replacement socket that came loose as well as a fracture or her acetabulum in the front and back.

To fix her problem, we fixed the posterior wall acetabular fracture with a plate and screws and then used a trabecular metal cup which had a solid fit plus multiple screws. Also used bone morphogenetic protein (BMP) to help recruit bone forming cells to the area to help her heal.

Thank you to the help of everyone who made this possible including nursing leadership, administrative leadership, anesthesia, physical therapy and all the support staff. .
