Our patient, N.C. (16 y.o.), was referred to us by the ophthalmologist for a myopia control plan using Orthokeratology. Considering the pattern of myopia progression, we opted for an active approach: an Ortho-K treatment with a custom lens design, which was developed and managed entirely in-house using dedicated software.

OD: Sphere -3.75, Cylinder -0.75 @ 145°
OS: Sphere -3,50 Cylinder -0.50 @ 40°

Short-Term Results: Efficacy Achieved in 7 Days!
The efficacy of the corneal reshaping process was rapid: after only one week of overnight wear, the patient achieved 20/20 (or 10/10) daytime visual acuity. This result validates the accuracy of both the fitting and the lens design applied.

The Primary Mission: While the excellent visual outcome provides immediate freedom, our main focus remains on blocking myopia progression. Our protocol includes rigorous monitoring every 3-6 months to verify the effective slowing (or halt) of the myopia advancement.

🇺🇸 Advanced Slit Lamp Evaluation Post-Ortho-K
The primary goal of our meticulous checks is twofold:

To prevent, as much as possible, the onset of potential complications ⚠️ related to the use of Orthokeratology (Ortho-K) lenses.
To intercept early 🔎 any detected alteration (for example, epithelial staining or an inflammatory reaction 🔥), directing the user immediately to the co-managing ophthalmologist 🩺 for further evaluations and the eventual management of the anomaly.

This surveillance by the optometrist works in synergy with and as a complement to the established standard medical eye monitoring.

Detailed Slit Lamp Evaluation 🔬
Our verification protocols using the slit lamp (Post-Ortho-K) are divided into two essential phases.

Phase 1: Evaluation Without Fluorescein 👀
This phase is important for the general assessment of the ocular condition in Ortho-K, designed to detect possible initial signs of stress before the instillation of any dye.

Observation of the Eyelids and Conjunctiva:

We check for signs of hyperemia (redness 🔴), particularly in the bulbar or limbal conjunctiva, which could indicate hypoxic stress or signs of inflammation related to contact lens wear.

Eyelid eversion can be performed to examine the tarsal conjunctiva and check for the possible presence of papillae or follicles, potential signs of an allergic or inflammatory reaction 🤧 to the maintenance solutions used for the lenses.

Corneal Integrity and Transparency:

Evaluate the stromal and endothelial transparency to rule out edema (swelling 💧) or non-inflammatory infiltrates (striae or folds), which could indicate a metabolic compromise.

Observe the corneal surface to detect any gross alterations or irregularities before applying fluorescein.

Lens Stability and Centration (Fitting Stability):

Although corneal topography is the reference instrument, the slit lamp allows for the visual observation of the correct static centration 🎯 of the treatment zone relative to the visual axis and verification of the absence of evident decentration.

Phase 2: Evaluation With Fluorescein ✨
Fluorescein highlights possible superficial cell anomalies and allows for the evaluation of the dynamic fitting of the Ortho-K lenses.
Epithelial Integrity and Staining (Epithelial Integrity and Staining): A drop of fluorescein can be used to identify epithelial staining. Specific patterns indicate potential fitting problems:
Central Staining: Often caused by a central air bubble 🫧 or excessive apical bearing.
Annular Staining: Typical of staining from debris accumulation 🦠 in the reverse curve zone or a suboptimal fitting that creates excessive contact in that area.
Diffuse Staining: May indicate generalized dry eye disease or lens manipulation trauma.
Tear Film/Fitting Assessment (Tear Film/Fitting Assessment): With the lens on the cornea, the fluorescein pattern must be carefully evaluated:
Look for optimal centration and a well-balanced bearing pattern (bull's-eye pattern 🎯).

We ensure there is adequate tear flow and exchange beneath the lens, especially at the edges.
A slit lamp check that does not include verification of epithelial integrity and observation of the conjunctiva is incomplete. Long-term safety in Ortho-K relies on these rigorous microscopic controls ✅.
Always implement the biphasic protocol (with and without fluorescein) and meticulously document ✍️ every finding, even the slightest.

Ruolo dell'Optometrista e la Co-gestione 🤝

Lo scopo principale delle nostre verifiche meticolose è duplice:

Prevenire 🛡️ quanto più possibile l'insorgenza di potenziali complicanze annesse all'uso delle lenti ortocheratologiche (Ortho-K).

Intercettare precocemente 🔎 qualsiasi alterazione rilevata (ad esempio, uno staining epiteliale o una reazione infiammatoria), indirizzando l'utente con la massima rapidità al medico oculista 🧑‍⚕️ che co-gestisce assieme a noi l'applicazione, per ulteriori valutazioni e l'eventuale gestione dell'anomalia.

Questa sorveglianza da parte dell'optometrista agisce in sinergia e a complemento del normale monitoraggio medico oculistico già stabilito.

Valutazione Dettagliata in Lampada a Fessura
I nostri protocolli di verifica in Lampada a Fessura (Post-Ortho-K) sono divisi in due fasi essenziali.

FASE 1: Valutazione senza Fluoresceina 💡

Questa fase è importante per la valutazione generale della situazione oculare in Ortho-K, per rilevare possibili segni iniziali di stress prima dell'instillazione di qualsiasi colorante.

Osservazione delle Palpebre e della Congiuntiva:

Verifichiamo non vi siano segni di iperemia (rossore) 🔴, in particolare nella congiuntiva bulbare o limbale, che potrebbe indicare stress ipossico o segni di infiammazione correlati al porto delle lenti a contatto.

Si può eseguire l'eversione palpebrale 🔄 per esaminare la congiuntiva tarsale e verificare la possibile presenza di papille o follicoli 🔬, possibili segni di reazione allergica o infiammatoria ai liquidi di manutenzione utilizzati per le lenti.

Integrità e Trasparenza della Cornea:

Valutare la trasparenza stromale ed endoteliale per escludere edema 💧 o infiltrazioni non infiammatorie (striae o folds), che potrebbero indicare una compromissione metabolica.

Osservare la superficie corneale per rilevare eventuali alterazioni macroscopiche o irregolarità prima di applicare la fluoresceina.

Stabilità e Centratura della Lente (Fitting Stability):

Pur essendo la topografia corneale lo strumento di riferimento, in lampada a fessura è possibile notare visivamente il corretto centraggio statico 🎯 della zona di trattamento rispetto all'asse visivo e verificare l'assenza di decentramento evidente.

FASE 2: Valutazione con Fluoresceina 🟢

La fluoresceina evidenzia possibili anomalie cellulari superficiali e permette di valutare il fitting dinamico delle lenti Ortho-K.

Integrità Epiteliale e Staining (Epithelial Integrity and Staining): Una goccia di fluoresceina può servire per identificare lo staining epiteliale. Modelli specifici ci indicano eventuali problemi di fitting:

Staining centrale ⭕: Spesso causato da una bolla d'aria centrale o da un eccessivo appoggio apicale (excessive bearing).

Staining ad anello 🍩: Tipico di uno staining da accumulo di detriti nella zona di inversione o di un fitting non ottimale che crea un contatto eccessivo in quell'area.

Staining diffuso ✨: Può indicare secchezza oculare generalizzata o trauma da manipolazione della lente.

Valutazione del Menisco Lacrimale/Fitting (Tear Film/Fitting Assessment): Con la lente sulla cornea, il fluorescein pattern deve essere valutato attentamente:

Cercare se avviene un ottimo centramento e un modello di appoggio ben bilanciato (bull's-eye pattern).

Ci assicuriamo che ci sia un adeguato flusso e scambio lacrimale 🌊 sotto la lente, soprattutto ai bordi.

Un controllo in lampada a fessura che non includa la verifica dell'integrità epiteliale e l'osservazione della congiuntiva è incompleto. La sicurezza a lungo termine nell'Ortho-K si basa su questi controlli microscopici rigorosi ✅.

Implementate sempre il protocollo bifasico (con e senza fluoresceina) e documentate minuziosamente 📝 ogni riscontro, anche il più lieve.
