Sapna Patel, Director of Clinical Services at Mosaic Pharmacy Service, was part of a panel discussion with Senator Tim Kaine. The discussion focused on prescription drug costs and the need for a value-based pharmacy like Mosaic.

The cost of drugs includes more than just what we pay for them.
· Studies have shown that 2 out of 3 seniors take 5 or more meds per day, and the cost of underuse, overuse, and mis-use of prescription drugs results in $530 billion per year in avoidable medical care.
· There will be 90 million doctor visits, ED visits, and hospitalizations for adverse drug reactions among seniors in this decade.
· It takes more than copay reductions to ensure seniors get the meds they need and get off the meds they don’t need, which is where Mosaic is having a positive impact.

This week’s featured Mosaic member is Betty, who was recommended to us to improve her medication regimen. Her medical history includes diabetes with neuropathy, atrial fibrillation, and chronic kidney disease. Her husband manages her medication and had been adjusting doses to avoid splitting pills. Prior to Mosaic, Betty was taking 18 prescribed and OTC medications.

Following her clinical review and home visit, our pharmacist found that Betty had an increased risk of bleed due to multiple therapies. Additionally, she was taking short-acting insulin and had symptoms of low blood glucose.

We educated Betty on hypoglycemia and coordinated with her prescribers to reduce her bleed risk and optimize her medication regimen. We also started Betty on Mosaic packaging to improve safety and simplicity.

As a result of Mosaic, Betty discontinued taking over-the-counter turmeric and discontinued duplicate diabetic medication which stopped her hypoglycemia episodes. After working with us, Betty and her husband are extremely pleased with her improved glucose control. Additionally, they love how our Mosaic medication box and synchronized monthly refills make it easier to manage her medications.

Meet Jessica, a Mosaic member who was referred to us because she needed help managing her medications. With conditions including high blood pressure, diabetes, insomnia, and depression, her medication regimen was complicated and overwhelming to manage alone.

After a Mosaic home visit and clinical review, our specialists found that Jessica had high blood sugar because she wasn’t adherent to her medication. Additionally, the prescribed sleep aid she was taking was not recommended for patients her age.

Our team worked with Jessica and her doctors to synchronize her medications each month, switch her to a safer sleep aid, and revise her diabetes treatment plan to include a less expensive combination pill and a healthier diet. As a result of our clinical recommendations, Jessica is saving $120 a month and is now adherent to her diabetes medication.

This week, we look at another Mosaic member's experience. Bill was referred to Mosaic after being hospitalized for chest pain and syncope. During his clinical review, our pharmacist found he had seven different providers and not everyone was aware of his complete medication regimen. In addition, Bob's caregiver was concerned over the number of medications he was taking and how difficult it was to communicate with all of his doctors.

Our clinical team helped facilitate improved coordination and communication among Bill's providers. As a result, Bill was able to discontinue unnecessary medications and he and his caregiver felt more confident and supported in his medication regimen. This week, we introduce Susan, a Mosaic member who was recommended to Mosaic after spending time in the hospital due to complications from pneumonia and sepsis. Upon discharge she was prescribed two antibiotics, however our team found that Susan was only prescribed a three-day supply of her medication when we conducted her initial clinical review.

Concerned by the potential safety and readmission risk, we worked directly with Susan’s primary care physician to obtain the necessary antibiotic orders and ensure she could receive a longer supply. As a result, Susan was able to stay healthy and avoid a potential hospital readmission due to a gap in her antibiotic therapy.
