Can’t say it enough…. Premolars. They can be the hardest teeth out there! This sneaky tooth was originally treated in 2022 and the patient had persistent discomfort and infection. Turns out there was another root whose canal bifurcated in the apical 1/3. Lots of pre curving of files to get this bad boy

⚠️ Long post! ⚠️ cliff notes version: crazy apico case, I love EndoCeramic Putty.

This was a wild case! Patient presented with a previously re treated #28 and recurrent infection. CBCT showed this tooth had a c shaped root with two root canal treated apices and an enormous periapical lesion! Immediately my spidy senses told me this was probably a periapical cyst. We chose to do apicoectomy. This was my first time using Endo Direct’s bioceramic putty and I was very pleasantly surprised! Other putties like to stick to my instruments rather than the tooth, but EndoCeramic was easy to manipulate, didn’t stick to my instruments, and didn’t wash out. A few days after the surgery I got the biopsy results back and my spidy senses were right! Periapical Cyst. Looking forward to seeing this patient back for re-evaluations
