Neonatal Jaundice
1. Greatest risk factors for NJ: Early gestational age; low birthweight
2. In breast feeding jaundice; continue breastfeeding and supplement 5 ml after each feed
3. An infant presenting with severe jaundice in first 24 hrs or after 1-2 weeks of birth, firstline tx is immediate phototherapy
Stress tests
1. Reasons to stress: angina; SOB/DOE; Chest pain; Arrhythmias; surgical risk assessment
2. Types: Stress EKG; Stress w/nuclear; Pharmacologic w/nuclear; Dobutamine; Stress Echo; NONE!
3. Normal EKG, symptoms: stress EKG
Pediatric Fluids
1. Maintenance: 100cc/kg (1st 10 kg); 50 cc/kg (11-20kg); 20 cc/kg (>20kg); D5-1/4NS + 20kcl
2. Fluid deficits (1 y/o): Mild: 3%; Moderate: 6%; Severe: 9%
3. Bolus 20cc/kg NSS over 20-30 mins
