Dr. Tara Dall is a board certified lipidologist with roots in family medicine and training at the University of Wisconsin Madison. She has spent the last 20 years using advanced biomarker testing in the clinical care of patients to prevent heart disease and diabetes. For 3 years she served as Chief Medical Officer of Virginia based Health Diagnostic Laboratory co-authoring research focused on using cutting edge laboratory medicine to identify disease years before traditional testing identifies it. Known for her passionate delivery of complex medical topics in patient-friendly terms, Dall is a sought after speaker, traveling the world to present research on advanced biomarkers. Just finished a fantastic week lecturing with Dr Wayne Peters. Lipids and lipoproteins, Apo B, Lp (a), sterols, advances in diabetes, metabolic syndrome, genomics and more. It was a delightful week on the Celebrity Apex (most beautiful ship I’ve ever sailed) . Lectures are always planned only on days we are “at sea” so plenty of time to visit the Norwegian Fjords. What a spectacular part of the world!. I am truly honored to have met such a wonderful group of clinicians. These courses at sea are a great way to earn CME and enjoy vacation at the same time.

I have a colleague and friend who has worked with the doctors and nurses in the ICU and ER. She, like many, now quarantined and wanting to help from home. She is willing to provide emotional and spiritual support during this unsettling time in our world.

Her words to ask each of us:

Are you in need of emotional, grief or spiritual support during this time of crisis? Are you asking yourself:

*Why is this happening to me and the world?
*How will I manage everything?
*How do I cope with all the emotions jumping around inside of me?
*What impact with this have on me and my family, our finances, our health?
*How will I find any meaning or hope during this crisis?
*What do I do next?

Emotional anguish, spiritual distress and physical suffering can all be reactions to events that surround us.

The new European 2019 lipid guidelines recommend high dose statins (and ezetimibe if needed) to aggressively lower LDL-C. Also it is recommended to measure lp (a) once. I am happy to see some acknowledgement of importance of lp (a) but seems counterintuitive here. High dose statins increase lp (a) and aortic stenosis risk! At least 30 % of patients have lp (a) elevation and higher % in the secondary prevention population. High dose statins also don't help hyperabsorbers of cholesterol (look at 4S study). That is 20 % of people. Shouldn't guidelines at minimum "do no harm"? Those of us who have been measuring lp (a) and Apo B or LDL P for decades now know how predictable that "statin induced increase" is in both hyperabsorpers and genetically elevated lp (a). I guess these and other guidelines just keep the "virtual lipid clinic" in business but it is unfortunate. I would love to see this disease trend reverse and would love to have a reason to retire. I suspect in 10 years this will get figured out by the decision makers. Until then we keep trying to discuss this when given the opportunity at medical education events.
